Job Summary:
We are seeking a compassionate and highly skilled Family Nurse Practitioner to join our healthcare team. Duties:
– Conduct comprehensive physical exams, including assessing patients’ vital signs and medical histories.
– Order and interpret laboratory tests and imaging studies as necessary to diagnose and manage patient conditions.
– Develop and implement individualized treatment plans for patients of all ages, from newborns to geriatric patients.
– Provide preventive care, including routine check-ups, health screenings, and vaccinations.
– Manage chronic conditions, such as diabetes, hypertension, and asthma, through medication management, lifestyle counseling, and patient education.
– Collaborate with healthcare teams to coordinate patient care, including referrals to specialists and other healthcare services.
– Utilize telehealth technology to provide virtual consultations and monitor patient progress remotely.
– Stay current with the latest developments in medicine and healthcare policy through ongoing education and training.
– Participate in case management to optimize patient outcomes and reduce healthcare costs.
– Provide occupational health services, including conducting physical exams and providing health education to employees.
Experience:
– Minimum of a Master’s degree in Nursing (MSN)
– Current licensure as a Family Nurse Practitioner in California
-Current DEA License in California
– Certification as a Family Nurse Practitioner (FNP) or the equivalent.
– A minimum of 2 years of experience as a Family Nurse Practitioner or in a related role.
– Experience providing telehealth services to patients.
– Experience working in a team-based healthcare environment.
-Bilingual, preferably Spanish.
