Job SummaryWe are actively seeking passionate Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to join our dedicated team across multiple locations, including Long Beach, East LA, and the San Gabriel Valley. This is an exciting opportunity to make a meaningful difference in patients’ lives by providing exceptional rehabilitative care in diverse settings such as outpatient clinics, inpatient facilities, skilled nursing facilities, home health environments, and acute care units. Duties

  • Assess patients’ physical conditions through detailed evaluations, including physiology and anatomy knowledge, to develop personalized care plans.
  • Implement therapeutic interventions such as trigger point therapy and other physiotherapy techniques to alleviate pain and improve function.
  • Educate patients on their conditions and recovery processes, ensuring they understand their treatment plans and discharge instructions.
  • Collaborate with interdisciplinary teams in inpatient, outpatient, orthopedics, geriatrics, pediatrics, and home health settings to optimize patient outcomes.
  • Document patient progress accurately using medical terminology and maintain detailed records for ongoing care and discharge planning.
  • Assist in developing and adjusting care plans tailored to individual needs across various environments like skilled nursing facilities and outpatient clinics.
  • Support discharge planning efforts by coordinating with healthcare providers and family members to ensure seamless transitions from hospital or facility to home or other care settings.

Experience

  • Prior experience working in inpatient settings, skilled nursing facilities, outpatient clinics or home health environments is highly preferred.
  • Demonstrated knowledge of physiology, anatomy, medical terminology, orthopedics, geriatrics, pediatrics, and acute care practices.
  • Skilled in providing patient care with a focus on pain management techniques such as trigger point therapy.
  • Experience working with diverse populations including elderly patients (geriatrics), children (pediatrics), or post-surgical orthopedic cases is advantageous.
  • Familiarity with care plans development, discharge procedures, and interdisciplinary team collaboration.
  • Previous exposure to outpatient therapy or clinic-based practice is desirable but not mandatory.
