Care Transition Coordinator
RN, PT, OT Home Health
Vertus HealthCare is hiring a driven Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) to serve as a Care Transition Coordinator. This is a relationship-focused, growth-oriented role for a clinician who thrives on building partnerships, driving referrals, and improving patient outcomes through seamless post-acute care transitions.
Responsibilities:
- Drive referral growth by building strong relationships with hospitals, health systems, inpatient facilities, and physician groups.
- Serve as Vertus HealthCare clinical liaison, representing our services and value in transitional care programs.
- Guide patients through safe, timely transitions from acute care to home health.
- Assess, plan, coordinate, and monitor patient-centered transition plans using evidence-based best practices.
- Promote adherence to post-acute care plans and ensure services are initiated and completed.
- Manage admission activity within an assigned territory while meeting or exceeding referral and admission goals.
- Conduct ongoing quality assurance visits with referral sources to support satisfaction and long-term partnerships.
- Track performance and outcomes while supporting growth initiatives.
What We’re Looking For:
- Licensed RN, PT, or OT in the state of employment.
- Graduate of an accredited nursing or therapy program.
- 2–3+ years of clinical experience, preferably in home health, hospice, or post-acute care.
- Strong sales mindset with the ability to influence, negotiate, and close referrals.
- Knowledge of transitional care best practices and post-acute regulations.
- Excellent communication, presentation, and relationship-building skills.
- Experience managing a territory, prioritizing leads, and executing sales plans.
- Self-motivated, results-driven professional who works independently.
- Previous healthcare sales or liaison experience preferred.
