iCare Health Services LLC is an integrated outpatient clinic delivering coordinated physical and behavioral health care, including primary care, mental health, substance use disorder treatment, crisis services, rehabilitation coordination, diagnostic testing, and care management.

We are seeking a compassionate and skilled FNP or PA to join our multidisciplinary team.
Position Overview
The FNP/PA provides comprehensive primary and integrated medical care in collaboration with behavioral health, psychiatric, and care coordination teams. This role supports chronic disease management, crisis evaluations (adults), pain management, opioid use disorder treatment, and diagnostic oversight in a fast-growing outpatient clinic.
Key Responsibilities
Primary & Integrated Medical Care
- Conduct physical exams and medical assessments
- Diagnose and treat acute and chronic conditions
- Order and interpret labs and diagnostics
- Prescribe medications within scope
- Provide preventive and wellness care
Behavioral Health Integration
- Collaborate with therapists and psychiatric providers
- Screen for mental health and co-occurring conditions
- Participate in multidisciplinary care planning
Crisis & Stabilization (Adults)
- Evaluate adults presenting in behavioral health crisis
- Perform risk assessments and medical clearance
- Coordinate escalation of care when needed
Substance Use & MAT Services
- Assess substance use disorders
- Prescribe/manage MAT when credentialed
- Support recovery-focused care
Care Coordination
- Manage transitions after hospitalization or crisis
- Coordinate specialty referrals and follow-ups
- Collaborate with case managers and health plans
Documentation & Compliance
- Maintain timely EHR documentation
- Follow HIPAA, 42 CFR Part 2, and payer standards
Qualifications
Required – FNP
- Active Arizona Nurse Practitioner license
- Board certification (Family NP)
- DEA registration & prescriptive authority
- CPR/BLS
Required – PA
- Active Arizona PA license
- NCCPA certification
- DEA registration
- CPR/BLS
Preferred
- Integrated or primary care experience
- Behavioral health or crisis exposure
- MAT experience
- Familiarity with AHCCCS or managed care
- Outpatient clinic background
