NP / PA Post Acute-Hospitalist Opportunity
We are currently seeking a Nurse Practitioner or Physician Assistant to join our team in Skilled Nursing Facilities. We have several opportunities in Port Charlotte, Venice and Lakeland Florida.This position is ideal for clinicians who enjoy internal medicine–focused care, value ongoing education, and want to make a measurable impact by reducing unnecessary hospital readmissions.
No SNF experience? We will train the right candidate through an extensive onboarding and education program.

Pay: $105,000.00 – $115,000.00 per year
