Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. Marin Community Clinics is looking for a mission-driven NP or PA Enhanced Care Provider to join our dynamic, multidisciplinary, Medical and Complex Care team to advance our goal of improving health outcomes for individuals with complex medical, behavioral health, and psychosocial needs. Our team is committed to increasing health equity among Marin’s most vulnerable patients through person-centered, trauma-informed care. Working collaboratively with Complex Care Case Managers, behavioral health, and other members of the clinical care team, the Enhanced Care Provider works as a member of an expert team of professionals to provide patient-centered, outcome- driven care that promotes independence and dignity.
To this end, The Enhanced Care Provider will provide specialized primary care services and support to a limited panel of patients with the goals of minimizing barriers to care, improving health outcomes, and helping patients increase capacity to manage their needs with greater independence. This may include street medicine, shelter-based primary care, and enhanced access to care (longer visits, walk-ins, home visits) for patients who struggle to meet their health goals within the traditional primary care model. The Enhanced Care Provider role is a patient-facing role that aims to build engagement and trust with our Complex Care patients. The Complex Care Provider will report to a Site Medical Director with additional support and collaboration from the Director of Complex Care.
We are looking for an NP or PA Provider who:
- Understands the interplay between chronic conditions and psychosocial issues.

- Is comfortable working with individuals with complex behavioral health presentations and/or substance use disorders.
- Has passion for helping individuals navigate a complex system of care.
- Is a team player and welcomes guidance from multidisciplinary team members.
- Demonstrates initiative, creativity, and enjoys problem solving.
- Is comfortable working with community partners including community-based organizations (CBOs), Marin County, medical/BH specialists, and insurance providers.
- Ascribes to the tenets of person-centered, trauma-informed, housing-first care.
- Demonstrate compassion and respect for individuals served in all interactions.
- Has excellent organizational skills.
- Demonstrates a willingness to be flexible and adaptable in all aspects of this work.
Bilingual English/Spanish language skills are highly desired.
This is a hybrid position with onsite and remote work schedule.
Responsibilities:
Manage a panel of complex patients by delivering specialized primary care strategies including street medicine. Lead MCC’s Enhanced Care Clinic efforts to stabilize high-risk patients using alternative care structures. Implement comprehensive patient care plan in partnership with patients. Triage and provide urgent care/walk-in visits. Provide health coaching and education to increase self-efficacy among target population. Lead/maintain key community partnerships (homeless shelters, homeless outreach agencies) Work with patients/case managers to address social determinants of health (SDOH) Track referrals and provide follow-up to ensure service delivery. Assist patients to coordinate all necessary care. Conduct home/community visits as needed. Consult/coordinate with interdisciplinary care team, including behavioral health, care navigation MAs, nursing, medical providers, and external partners. Document all Patient interactions and services using MCC’s electronic health record AND secondary database. Attend internal and external meetings (i.e. case conferencing, administrative). Attends trainings such as health coaching, engagement techniques, etc. Timely and accurate reporting of data as requested to MCC CM programs. May travel between clinics or meet patients in the community as needed. Provide services to satellite clinics as needed; may be asked to work variable schedules. during extended-hour clinics (e.g., 10am-7pm on one or more days).

Qualifications:
Education and Experience:
Advanced Practice Provider (NP/PA) from an accredited medical school or training program; current California medical license and DEA certificate required.
1-2 years’ experience (and passion for) providing support to individuals with chronic and complex BH needs, complex medical conditions and/or substance-using individuals highly preferable.
Experience and familiarity with housing resources/housing navigation in Marin is highly preferable.
Knowledge of state regulations and legal/ethical standards related to patient rights, and client/patient confidentiality required.
Training in motivational interviewing highly desired/or willingness to learn.
Demonstrated ability to be proactive, flexible, and multitask in a busy clinical environment.
Willingness to receive regular feedback and problem solve in real-time.
Experience with EHR systems (EPIC preferred).
Excellent written and verbal communication skills; commitment to diversity, equity, inclusion, and trauma-informed care principles.
Active BLS Certification.
Required Skills and Abilities [
Must be fluent in English with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
Ability to write reports and business correspondence in English.
Highly desired: ability to communicate with patients and partners in Spanish: effectively present information and respond to questions from clients, customers, and the general public in Spanish.
Strong preference for a candidate than is able to conduct clinical assessment interviews and case management interventions with patients in Spanish.
Computer Skills:
To perform this job successfully, an individual must be computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.

Valid California Driver’s License
Physical Requirements
Prolonged standing, walking, and prolonged periods sitting at desk and working on a computer.
Fulfill immunization and fit for duty regulatory requirements.
Must be able to lift up to 25 pounds at a time and move medical equipment as needed.
Fine motor hand movements typical of performing standard medical provider job duties.
Able to read charts graphs and readings on medical equipment.
Specific vision abilities required by this job include close vision, color vision, depth perception and the ability to adjust focus.
