Nurse Practitioner or Physician Assistant – Advanced Primary Care

Location: Munster, Indiana

Job Type: Full-time

An innovative and well-established primary care group in the Munster, Indiana area is seeking a motivated Nurse Practitioner to join its growing team. This is a unique opportunity to practice at the forefront of a tech-enabled, data-driven, and team-based care model. The incoming provider will manage a panel of adult patients with a focus on longitudinal chronic disease management, working collaboratively with a multidisciplinary team to deliver high-quality, value-based care.

Our practice is built on a foundation of providing comprehensive, high-touch care that goes beyond traditional primary care. We leverage remote patient monitoring and a proprietary data platform to proactively manage patient health and optimize outcomes. This role offers the opportunity to work in a supportive and collaborative environment where providers are empowered to build long-term relationships with their patients and make a meaningful impact on their health.

We are seeking a candidate who is passionate about primary care and excited by the prospect of working in an advanced practice setting. The ideal candidate will be a tech-savvy, statistically-minded provider with a strong interest in chronic disease management. We are open to experienced nurse practitioners as well as new graduates who have a strong RN background in high-acuity settings such as ICU or cardiovascular units.

Key Responsibilities

  • Provide comprehensive primary care to a panel of adult patients (18+).
  • Manage chronic diseases through a data-driven, team-based approach.
  • Utilize remote patient monitoring and other technologies to optimize patient care.
  • Collaborate with a multidisciplinary team of providers, medical assistants, dietitians, and other support staff.
  • Participate in a gradual onboarding process with a focus on quality over volume.

Qualifications

  • Master of Science in Nursing (MSN) from an accredited program.
  • Current, unrestricted Nurse Practitioner license in the state of Indiana.
  • 3+ years of experience as a Nurse Practitioner in a primary care setting is preferred.
  • New graduates with a strong RN background in ICU, cardiovascular, or other high-acuity settings are encouraged to apply.
  • Experience with remote patient monitoring and other healthcare technologies is a plus.
  • Strong interest in chronic disease management and a team-based approach to care.

Benefits

We offer a highly competitive compensation and benefits package designed to attract top talent, including:

  • Competitive salary of $115,000 – $135,000, commensurate with experience.
  • Annual bonus potential of up to $7,500.
  • 401(k) with a 3.5% company match after six months of employment.
  • Three weeks of paid time off (four weeks after five years).
  • Three paid holidays.
  • United Healthcare medical insurance with 50% of the premium covered by the employer.

Schedule

  • Full-time, 40-hour work week.
  • 36 hours of patient care and 4 hours of administrative time.
  • One late day per week (11:00 AM – 7:00 PM).
  • One Saturday per month (9:00 AM – 1:00 PM), with a half-day on the preceding Friday.
