Your Purpose in This Role
As a Nurse Practitioner or Physician Assistant, you will deliver expert wound care while building meaningful relationships with patients, families, and care teams. You will have the autonomy to practice thoughtfully and the support of a collaborative, mission-driven team.

What You’ll Do:Provide Compassionate, Advanced Wound Care
- Conduct on-site wound assessments, treatment, and follow-up in homes, post-acute facilities, and community settings.
- Deliver evidence-based, advanced wound care therapies with precision and compassion.
Collaborate with Heart and Excellence
- Work closely with physicians, nursing teams, and facility staff to ensure continuity of care.
- Participate in interdisciplinary care planning to reduce complications and rehospitalizations.
Educate and Empower
- Teach patients and caregivers about wound care, healing, and prevention.
- Provide reassurance, clarity, and encouragement throughout the healing journey.
Document with Integrity
- Use EMR systems to document patient encounters, treatment plans, and progress accurately and efficiently.
Who You AreRequired Qualifications
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Alabama.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Valid driver’s license and willingness to travel locally.
Preferred (But Not Required)
- Home health, post-acute, or wound care experience.
- A passion for working with older adults and complex patients.
