Prospect Healthcare S C is a physician practice management company specializing in in-home primary care for geriatric and disabled patients. We proudly serve primarily Medicare patients who value compassionate, comprehensive care delivered in the comfort of their homes.
We are currently seeking a Nurse Practitioner or Physician Assistant to join our growing team and provide high-quality, patient-centered care throughout Chicago and surrounding suburbs.
Responsibilities
- Communicate effectively with patients, caregivers, agency nurses, providers, and vendors
- Assess complex patients using appropriate diagnostic methods
- Implement evidence-based interventions and care plans
- Promote preventative care and early intervention
- Perform minor procedures (e.g., wound care, debridement) as needed
- Evaluate medications and prescribe treatments as appropriate
- Demonstrate compassion, empathy, and active listening
- Complete thorough and timely documentation in the EHR
- Sign care plans, nursing orders, DME orders, and required paperwork promptly
Education & Experience Requirements
- Master’s Degree in Nursing (NP) or PA program completion
- Minimum 1 year of clinical experience
- Active Illinois NP or PA license and board certification
- BLS and ACLS certification
- Active NPI number and DEA
- Valid driver’s license, auto insurance, and reliable transportation
- Willingness to travel to patient homes
Required Skills & Preferred Experience
- Proficient with EHR documentation
- Strong computer skills (Word, Excel, PowerPoint)
- Excellent verbal and written communication skills
- Experience in:
- Family Practice
- Geriatrics
- Palliative and end-of-life care
- Motivational interviewing
- Ability to diagnose and manage chronic and urgent conditions in older adults
- Home care experience preferred
- Strong clinical judgment and patient-centered approach

Job Types: Full-time, Part-time
Pay: $50.00 – $70.00 per hour
