Due to continued growth, we are seeking a bilingual Family Nurse Practitioner or Physician Assistant to join our team under a fee-for-service model.
Position Summary
The Family Nurse Practitioner (FNP) or Physician Assistant (PA) will provide comprehensive primary care services in a busy outpatient setting. The ideal candidate is a bilingual (English/Spanish), patient-centered clinician who enjoys working with multicultural communities and is open to learning more about integrative and functional medicine.
Key Responsibilities
- Collect subjective data by obtaining complete and focused patient histories.
- Collect objective data by performing thorough physical examinations.
- Develop assessments and diagnoses for common medical conditions and manage chronic illnesses.
- Create and implement management and treatment plans, ensuring accurate and timely progress notes and continuity of care.
- Facilitate referrals to appropriate specialists, health facilities, agencies, and community resources.
- Order and manage pharmacologic interventions; assist with medical chart review for follow-up and quality assurance.
- Develop and implement patient education plans; provide counseling on treatment adherence, lifestyle changes, and health maintenance.
- Maintain complete and compliant medical records for all patients, including documentation of follow-up and referrals.
- Meet regularly with supervising physician(s) to review charting and medical care provided.
- Meet productivity standards as defined by the Chief Medical Officer and senior leadership.
- Participate in quality improvement initiatives.
- Apply or be willing to learn integrative and functional medicine concepts (knowledge in this area is a plus).
Qualifications
Experience
- To be considered for the position, the candidate must have a minimum of 2 years’ experience working full-time as an NP/PA in an outpatient primary care clinical setting
- Must be comfortable working independently
Education
- Graduate of an accredited Nurse Practitioner (NP) program and RN program
- or graduate of an accredited Physician Assistant (PA) program.
- Master’s Degree in Nursing (for NPs).
Required for All Candidates
- National Provider Identifier (NPI)
- Basic Life Support (BLS) certification
- Current DEA Registration Number
- If hired as a new graduate NP or PA, the California Furnishing Number and DEA registration (if applicable) must be obtained within 12 months of hire. In this case, the probationary period may be extended from six (6) to twelve (12) months.
