This PA-C role is perfect for a clinician who enjoys autonomy, hands-on medicine, and becoming a familiar, trusted presence for patients and staff. You’ll balance primary care and wound management while practicing in an environment that values relationships over volume and quality over quotas.
Your Role
- Provide on-site primary care and wound management within senior living communities
- Manage chronic conditions, acute concerns, and preventive care for geriatric patients
- Round consistently in assigned communities to build long-term patient relationships
- Collaborate closely with nursing staff, therapy teams, families, and leadership
- Ensure timely documentation, follow-up, and continuity of care

What We’re Looking For
- Active Minnesota PA-C license
- Experience or strong interest in geriatrics, internal medicine, or wound care
- Comfortable working independently while collaborating within a team model
- Reliable transportation for local community travel
