SpringCare Clinic is expanding its chronic wound care program across skilled nursing facilities, long-term care communities, assisted living and memory care settings, as well as select patient homes throughout the Dallas–Fort Worth area. We are seeking an experienced Nurse Practitioner or Physician Assistant to join our growing wound care team.
This role offers immediate patient volume with continued growth as additional facilities onboard. It is well suited for clinicians who value flexibility, clinical autonomy, and the opportunity to focus on high-quality wound management in a structured, supportive program.
Providers work in a mobile setting, delivering longitudinal care to patients with complex and chronic wounds while collaborating closely with facility nursing staff to ensure continuity and optimal outcomes. You will practice at the top of your license, performing comprehensive wound evaluations, treatment planning, debridement, and advanced wound care therapies, including biologic graft applications when clinically appropriate.
Responsibilities
- Conduct wound assessments, measurements, and staging
- Develop and manage individualized treatment plans
- Perform conservative and sharp debridement
- Apply advanced wound care products and biologic grafts when indicated
- Complete timely and accurate EMR documentation
- Communicate with facility care teams regarding patient progress
Qualifications
- Active and unrestricted Texas NP or PA license
- Wound care experience preferred
- Debridement proficiency required
- Wound care certification preferred (not required)
- Reliable transportation
- Current BLS certification
Compensation
$125–$175 per patient visit (based on complexity)
