Full Job Description

TrustedCare Medical Group is seeking a dedicated and skilled Nurse Practitioner or Physician’s Assistant to serve as a field-based neuyrology provider in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs). This role focuses on delivering on-site neurology services to patients with complex neurological conditions in post-acute and long-term care settings. The provider will work independently in the field while collaborating closely with supervising neurologists, facility staff, and the broader care team to ensure high-quality, patient-centered neurological care.

Key Responsibilities: Patient Care & Clinical Duties

  • Perform comprehensive neurological assessments on residents in SNFs and ALFs, including detailed medical histories, neurological examinations, and functional/cognitive evaluations.
  • Diagnose and manage acute and chronic neurological conditions common in post-acute and long-term care populations (e.g., dementia, stroke, Parkinson’s disease, seizures, neuropathies, migraines, multiple sclerosis).
  • Order, review, and interpret relevant diagnostic studies, coordinating with hospitals, imaging centers, and laboratories as needed.
  • Prescribe and manage medications, including controlled substances, within licensure and scope of practice.
  • Develop individualized care plans that align with each patient’s condition, goals of care, and facility protocols.
  • Provide education to patients, families, caregivers, and facility staff on neurological conditions, medication regimens, behavioral strategies, and disease progression.
  • Support transitional care for patients discharged from hospitals to SNF/ALF settings, ensuring continuity of neurological care.

Facility & Team Collaboration:

  • Collaborate with nursing staff, therapists, and medical directors in facilities to provide coordinated and comprehensive care.
  • Communicate effectively with family members and facility care teams regarding diagnoses, treatment options, and prognosis.
  • Participate in interdisciplinary care meetings, case reviews, and family conferences as required.
  • Serve as a clinical resource for facility staff on neurological best practices and patient management.

Administrative & Compliance:

  • Document all visits, assessments, and care plans in the EHR accurately and in compliance with state, federal, and facility requirements.
  • Ensure adherence to facility-specific protocols, regulatory compliance, and Arizona state practice laws.
  • Assist in developing and implementing neurology protocols for SNF/ALF patients to standardize and improve care delivery.
  • Maintain strong communication with trustedcare medical group leadership regarding patient progress, facility updates, and operational needs.

Qualifications:

Education & Licensure:

  • Graduate of an accredited Physician Assistant (PA-C) or Nurse Practitioner (NP) program.
  • Current and unrestricted Arizona state license as a PA-C or NP.
  • Board certification by NCCPA (PA-C) or a national nursing certifying body (NP).
  • DEA registration and ability to prescribe controlled substances in Arizona.

Experience:

  • Minimum 1–2 years of neurology, internal medicine, or geriatrics experience (preferred).
  • Prior experience in SNF, ALF, post-acute, or long-term care settings strongly preferred.
  • New graduates with relevant neurology or geriatric rotations will be considered with mentorship.

Skills & Competencies:

  • Strong diagnostic and clinical reasoning skills specific to geriatric and neurologic care.
  • Ability to manage complex patients with multiple comorbidities.
  • Excellent communication skills with patients, families, and interdisciplinary teams.
  • Adaptability to work independently in varied facility environments.
  • Proficiency with EHR systems and mobile documentation tools.

Job Types: Full-time, Part-time, Contract

Pay: $120,000.00 – $150,000.00 per year
