You’ll serve as a trusted clinical partner inside senior living communities, blending primary care and wound management in a setting built for thoughtful, unrushed medicine.
What Your Days Look Like
- Deliver on-site primary care to seniors in assisted living, memory care, and independent living
- Provide hands-on wound assessments and treatment
- Manage chronic conditions while focusing on prevention and early intervention
- Build strong, consistent relationships with residents, nursing staff, and families
- Communicate clearly with interdisciplinary teams to ensure continuity of care
- Document thoroughly with an emphasis on follow-through and outcomes

What We’re Looking For
- Active Minnesota PA-C license
- Experience or strong interest in geriatrics, internal medicine, or wound care
- Comfortable practicing independently within a collaborative care model
- Strong communication and relationship-building skills
- Reliable transportation for local community travel
