Full Job Description

Nurse Practitioner (NP) / Physician Assistant (PA) – Full-Time

We are actively hiring experienced Nurse Practitioners (NPs) / Physician Assistants (PAs) to provide comprehensive care across private Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALF) throughout the Long Island area.

Responsibilities

  • Conduct thorough patient assessments and develop individualized care plans
  • Diagnose, manage, and treat both acute and chronic conditions
  • Monitor patient progress and adjust treatment plans as needed
  • Document all visits using Electronic Health Record (EHR) systems
  • Collaborate with physicians, nursing staff, and interdisciplinary teams
  • Educate patients and families on health management and wellness strategies
  • Maintain compliance with all healthcare regulations and HIPAA standards

Skills & Competencies

  • Strong diagnostic and clinical judgment skills
  • Proficient in medication management and post-acute care protocols
  • Familiarity with infection prevention and chronic disease management
  • Excellent communication, teamwork, and patient education skills
  • Highly organized and efficient in managing multiple patients daily
  • Comfortable working independently within facility settings

Qualifications

  • Licensed and Board-Certified NP or PA in New York State
  • Authorized to work in the United States
  • Previous experience in Skilled Nursing, Rehabilitation, or Long-Term Care (preferred)
  • Strong work ethic and commitment to high-quality care
  • Proficiency with EHR systems (e.g., Practice Fusion, PointClickCare, or similar)
  • Fluent in English (additional languages a plus)
