Your Role.
- Provide on-site medical assessments, primary care, and wound management.
- Manage chronic conditions and preventive care for senior patients.
- Round consistently within assigned communities to build continuity and trust.
- Collaborate closely with nursing staff, therapy teams, and families.
- Ensure timely, thorough documentation and follow-up care.
