Key Responsibilities.
- Conduct in-person medical assessments and follow-up visits within senior communities.
- Provide wound care and manage chronic and acute conditions.
- Partner closely with nursing staff, families, and interdisciplinary teams.
- Educate patients and caregivers on treatment plans and preventive care.
- Complete thorough documentation to support continuity and outcomes.

Qualifications.
- Active Minnesota Physician Assistant license.
