Healthy Home Primary Care serves patients in both an outpatient primary care setting and inpatient rehabilitation centers.

Job Summary

We are seeking a highly skilled and compassionate Physician Assistant to join our healthcare team. The PA will deliver comprehensive healthcare services to patients at our outpatient primary care clinic and affiliated rehabilitation center. The role involves caring for a diverse patient population, the majority being geriatric, with an emphasis on promoting health, managing chronic conditions, and facilitating rehabilitation.

Key Responsibilities

  • Perform patient assessments, including medical history, physical examination, and diagnostic test interpretation.
  • Diagnose, treat, and manage acute and chronic medical conditions for patients of all ages, with an emphasis on geriatric care.
  • Develop and implement individualized care plans that incorporate preventive care, rehabilitation, and health maintenance.
  • Prescribe and administer medications, treatments, and therapies, considering the specific needs and conditions of each patient.
  • Perform necessary procedures, including but not limited to joint injections, wound care, incision and drainage, laceration repair, ear irrigation, cervical cancer screenings, and simple excisions.
  • Order and interpret diagnostic tests, such as laboratory and imaging studies, to aid in diagnosis and treatment planning.
  • Collaborate with other healthcare professionals, including physicians, nurses, physical therapists, and occupational therapists, to optimize patient care.
  • Provide patient education on health promotion, disease prevention, rehabilitation, and chronic disease management.
  • Maintain accurate and up-to-date patient records in accordance with healthcare standards and regulations.
  • Stay current with evidence-based practices, guidelines, and healthcare advancements through ongoing education and professional development.

Qualifications

  • Current FL licensure as a Physician Assistant.
  • Strong clinical and diagnostic skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and collaboratively within a healthcare team.
  • A passion for providing patient-centered care and a dedication to promoting the health and well-being of patients, particularly in the geriatric population.

Pay: $110,000.00 – $125,000.00 per year
