Job Summary
We are seeking a compassionate and experienced Nurse Practitioner (NP) to provide high-quality medical care to residents in a nursing facility setting. This role focuses on patient-centered care, chronic disease management, and collaboration with an interdisciplinary healthcare team to ensure optimal outcomes for our residents.
The ideal candidate is clinically strong, organized, and comfortable working independently while maintaining close communication with physicians, nursing staff, and administration.
Key Responsibilities
- Provide comprehensive medical assessments, diagnoses, and treatment plans for nursing facility residents
- Manage chronic and acute conditions, including medication management and follow-up care
- Perform physical exams, order and interpret diagnostic tests, and document care appropriately
- Collaborate with physicians, nursing staff, therapists, and facility leadership
- Participate in care plan meetings and interdisciplinary team discussions
- Ensure compliance with federal, state, and facility regulations
- Educate residents, families, and staff on care plans and health conditions
- Maintain accurate and timely medical documentation
Qualifications
- Current Nurse Practitioner license in the state of practice (required)
- National NP certification (AANP or ANCC)
- DEA license (or ability to obtain)
- Experience in long-term care, skilled nursing, geriatrics, or post-acute care preferred
- Strong clinical judgment, communication, and organizational skills
- Ability to work independently and as part of a healthcare team

Pay: $125,000.00 – $150,000.00 per year
