Job OverviewUrbancare, LLC is currently hiring for a full-time Nurse Practitioner or Physician Assistant to join our growing physician owned Internal Medicine/Pediatrics practice located in Lake In The Hills, IL.
Qualifications
- Licensed Physician Assistant/Certified Nurse Practitioner (PA-C, APRN, FNP-C)
- Internal Medicine or Family Practice experience required
- Proficiency in ordering and interpreting diagnostic tests
- Strong ability to perform physical examinations, diagnose and treat medical conditions, manage patient care plans, and document patient information accurately
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a healthcare team
- Strong critical thinking and problem-solving abilities to address complex health issues.
Responsibilities
- Conduct thorough patient assessments, including medical history and physical examinations.
- Develop and implement individualized treatment plans based on patient needs and clinical guidelines.
- Provide education to patients regarding their health conditions and treatment options.
- Document all patient interactions accurately in medical records to ensure continuity of care.
- Collaborate with physicians and other healthcare professionals in managing complex cases.

- Outpatient care only
- One Saturday morning per month is required
- Salary commensurate with experience
