Full Job Description

NOELA Community Health Center, NOELA CHC, is a community health center and Federally Qualified Health Center that provides primary medical care to patients in New Orleans East and the surrounding area. The clinic focuses on accessible and culturally responsive health care to Vietnamese, Black, Latinx, and other residents of the neighborhood.

The bilingual Nurse Practitioner or Physician Assistant provides primary care services to adult patients with acute and chronic medical conditions. You work in an outpatient setting and deliver direct patient care in collaboration with a primary care physician and the broader health care team. You perform physical examinations, develop assessments and treatment plans, order and interpret diagnostic tests, and provide medical counseling and education to patients and their families.

In this role you see patients of all ages who come to NOELA CHC. You conduct comprehensive medical evaluations, manage common chronic conditions, and address acute health concerns. You use screening and diagnostic tests when needed, recognize abnormal findings, and create clear follow up plans. You document all patient encounters in a timely and accurate way in the electronic health record, which is required for quality patient care and compliance.

You provide ongoing patient education during visits and contribute to group health education activities when needed. You may provide brief nurse case management for patients with complex health needs. You help triage patients in the clinic and by telephone based on the urgency of their symptoms. You keep the supervising or collaborating physician informed about unusual conditions or patient reactions and work closely with medical assistants, nurses, and other staff to support safe and effective patient care.

You participate in clinic wide quality improvement efforts. You assist with continuous quality improvement projects that focus on better health outcomes, patient experience, and efficient care delivery. You help identify community and health resources that can support patients and their families and you guide patients in how to use those resources. You also bring ideas for new or improved community resources and care patterns that align with the mission of NOELA CHC and MQVN CDC.

As a member of the care team you cross train with other staff when needed to assist with tasks related to patient care. You show a willingness to learn new skills, ask questions before starting unfamiliar assignments, and take part in continuing medical education activities to maintain and strengthen your clinical skills.

Minimum qualifications

  • Master’s degree in nursing with a specialization in family care, or an equivalent graduate degree for a Physician Assistant

  • Current licensure and prescriptive authority, with certification by the State Board of Medical Examiners

  • National board certification

  • Ability to use standard computer applications and an electronic health record

  • Strong oral and written communication skills

  • Ability to work in a culturally diverse, patient centered, holistic environment

  • Must be able to work in a diverse setting

Preferred qualifications

  • At least one year of full time experience as a clinical nurse practitioner or physician assistant in a medical setting

  • Experience in primary care, community health, or a Federally Qualified Health Center

  • Bilingual fluency in Vietnamese
