Seeking a highly skilled and experienced Physician Assistant (PA) or Nurse Practitioner (NP) to join our dynamic and specialized team. This is a unique opportunity to utilize both critical care and nephrology expertise in a comprehensive role spanning inpatient, outpatient, and dialysis settings.
The ideal candidate must have proven experience managing critically ill patients in an intensive care setting and providing full-spectrum care for patients across all stages of kidney disease.
Key Responsibilities and Scope of Practice
The selected APP will operate with a high degree of autonomy and collaboration, managing complex patient populations.
Clinical Responsibilities
- Critical Care Management: Manage critically ill patients in the Intensive Care Unit (ICU) demonstrating proficiency in acute care protocols and procedures.
- Comprehensive Nephrology Care: Provide expert care for patients with all stages of Chronic Kidney Disease (CKD), Acute Kidney Injury (AKI), complex electrolyte and acid-base disorders, and refractory hypertension.
- Patient Assessment & Diagnosis: Conduct thorough physical examinations, record detailed health histories, and accurately diagnose both acute and chronic medical problems.
- Treatment Planning & Medication Management: Develop and implement evidence-based, individualized treatment plans, including the judicious prescribing and management of complex medication regimens and non-pharmacologic therapies.
- Dialysis Oversight: Oversee patient care in various dialysis modalities (hemodialysis, peritoneal dialysis), provide comprehensive education on treatment options, and actively participate in patient care plan meetings.
- Post-Hospital & Transitional Care: Ensure smooth transitions of care by performing timely follow-up for patients recently discharged from the hospital.
Collaborative and Administrative Duties
- Interdisciplinary Collaboration: Work closely and effectively with our team of nephrologists, attending physicians, hospitalists, registered nurses, dietitians, and social workers.
- Patient & Family Education: Serve as a primary educator and counselor, providing clear and compassionate education to patients and their families regarding lifestyle modifications, disease management, and prognosis.
- Diagnostic Interpretation: Independently interpret laboratory results, radiologic studies (X-rays, CT scans), and other diagnostic reports to inform patient care decisions.
- Documentation & Compliance: Maintain accurate, complete, and timely patient records and documentation of clinical rounds, adhering to all regulatory standards.
- Quality Improvement: Actively participate in clinical quality improvement projects, case reviews, and patient safety initiatives.
- Referral Management: Initiate appropriate and timely referrals to other medical and surgical specialists as necessary.
