The Palliative Care APP supports the palliative care program at Highland Hospital including rounding daily rounding and writing progress notes on current patients, gathering preliminary information and completing initial database for new consults and admissions, assisting with discharge planning, data collection and management with Palliative Care Administrative Assistant, coordinates consults/family meetings/team meetings, coordinates and orients trainees, assists with bereavement cards, participates with quality improvement, and documents interdisciplinary care team meetings. The APP is part of the Palliative Care Interdisciplinary Team (PC IDT), the Palliative Care Clinical Quality Service Committee (PC CQSC), and part of the Joint Commission (JC) Advanced Certification Team. The APP participates with staff orientation. As time permits, the APP is also expected to be the liaison between palliative care and the ICU, oncology, and gyn-oncology services. This position will carry important responsibilities in gathering and interpreting quality data which will be tied to our patient care and satisfaction outcomes and eventually play into our reimbursement. Assuring the quality and integrity of clinical data will be an important aspect of this job. Salary Range: $98,000.00 – $130,083.20 annually The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Job Requirements

  • Education: Fulfilled the educational requirements to be an NP or PA in Adult, Family, or Geriatrics
  • Experience: 4+ years of palliative care, geriatrics, general medicine, or equivalent
  • License/Certification Required: NP (national certification) or RPA-C
  • Skills: Excellence in written and spoken communication. Sufficient computer skills for using eRecord, e-mail, and basic data skills (Excel, Word). High level of independence, professionalism, and organization. Ability to cope well with death, dying, and suffering.
  • Employment conditional on satisfactory credentialing and staff appointment to Highland Hospital
