Job Description
Bella Health Group provides specialized wound care using the latest advances in treatment and a holistic, patient-centered approach. Our team of trained professionals leverages data, analytics, AI, and the latest in wound management to create personalized plans so patients heal safely and effectively.
We are looking for a Wound Care NP/PA to join our team in Sacramento, CA to provide patient care for adults with acute and chronic wounds in outpatient and facility-based settings. This role is responsible for assessing, diagnosing, and managing wounds, performing wound-related procedures, and educating patients and families, in collaboration with the supervising physician and the Bella Wound Care team.
Essential Duties & Responsibilities
· Conduct focused histories and physical examinations for patients with pressure injuries, diabetic foot ulcers, venous stasis ulcers, surgical wounds, and other complex wounds.
· Perform wound-related procedures as allowed by scope and training (e.g., debridement, advanced dressing application, negative pressure wound therapy set-up, compression wraps).
· Develop and implement individualized wound care treatment plans in collaboration with the supervising physician.
· Order and interpret appropriate diagnostic studies related to wound care (e.g., labs, imaging, vascular studies) when clinically indicated.
· Monitor wound healing progress, identify complications, and adjust treatment plans as needed.
· Provide patient and caregiver education on wound care techniques, off-loading/pressure relief, infection warning signs, and prevention strategies.
· Document clinical encounters in the electronic medical record in a timely and accurate manner, consistent with organizational and regulatory standards, and assist with billing compliance.
· Communicate and coordinate care with facility staff, referring providers, and other members of the interdisciplinary team.
· Maintain compliance with infection control policies, HIPAA, and all applicable state and federal regulations.
· Participate in ongoing quality improvement activities and adhere to Bella Wound Care clinical protocols and guidelines.
· Assist in improving the wound management workflows with the latest information and proven practices.
· Assist in recruitment and training of new team members, and generally support the expansion of our team.
QualificationsRequired
· Current, unrestricted California Nurse Practitioner (NP) license or California Physician Assistant (PA) license.
· National certification as NP (AANP/ANCC) or PA (NCCPA).
· Minimum 2 years of clinical experience as an NP or PA (wound care, surgery, vascular, podiatry, hospitalist, or similar setting preferred).
· Demonstrated competency in wound assessment and basic wound management (measurement, staging, tissue evaluation, dressing selection).
· Current BLS certification.
· Valid driver’s license and reliable transportation for local travel within the Sacramento area.
· Strong communication, clinical judgment, and documentation skills; ability to work both independently and within a multidisciplinary team.
Preferred
· Formal wound care certification (e.g., CWCN, CWS, WCC or equivalent).
· 3+ years of dedicated wound care experience (outpatient wound clinic, SNF rounds, home or mobile wound care).
· Experience working in skilled nursing facilities, assisted living, or long-term care environments.

Position Type & Compensation
This is an independent contractor (1099) position, paid hourly at a rate of $60-$95 depending on credentials and experience, with the possibility to convert to a W-2, salaried employee position. We are looking for individuals that have time flexibility, and can work for at least 2 days per week, as part of a mobile wound care service to assisted living and skilled nursing facilities in the Sacramento, CA area.
