Location: Antioch, CA
We are a multispecialty medical group seeking an Endocrinology and Primary Care Nurse Practitioner/Physician Assistant to provide full spectrum primary and preventative care. The Physician Assistant shall be responsible to provide follow-up care to patients with diabetes and endocrine disorders with attention to medication management for glucose, blood pressure and cholesterol control; other preventive care for diabetic patients; and communication with primary care providers. Will develop expertise in the management of patients with diabetes and other endocrine disorders Demonstrates a fundamental grounding in nursing/medical practice with mastery of knowledge within a defined content area. Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings.
Responsibilities: Prescribe all necessary medications and treatments
Perform comprehensive physical examinations of patients Develop and implement patient management policies and procedures Facilitate referrals to other healthcare professionals and medical facilities Communicate with collaborating physician or specialist regarding patient care
Qualifications: Previous experience in nursing or other medical fields Ability to build rapport with patients Ability to thrive in a fast-paced environment Excellent written and verbal communication skills Strong leadership qualities
Education: Physician Assistant Masters of Science Degree, Graduate of accredited Physician Assistant Program Certification Current CA licensure
Preferred Experience: Outpatient practice experience focuses on the primary care needs of adult and older patients. Experience with Electronic Medical Record
