Job SummaryWe are seeking a Physician Assistant or Nurse Practitioner to join our physiatry (Physical Medicine & Rehabilitation) team in skilled nursing and rehabilitation facilities.

If you are new to SNFs, rehab, or even a newer graduate, we are happy to train you. You’ll work closely with an experienced physiatrist and a supportive interdisciplinary team, helping residents improve function, prevent complications, and return to their highest level of independence.

Responsibilities

  • Perform admission, follow-up, and discharge visits with attention to functional status, mobility, and rehab goals.
  • Diagnose and manage common post-acute and chronic conditions (e.g., debility, orthopedic/post-surgical cases, neurologic conditions, and medical comorbidities).
  • Modify rehabilitation and medical treatment plans in collaboration with the supervising physiatrist, PT/OT/SLP, nursing, and social work.
  • Help identify and address falls, acute changes in condition, wounds, pain, and infections in the SNF/rehab setting.
  • Participate in interdisciplinary team (IDT) meetings and care conferences as needed.
  • Educate patients, families, and staff on rehab goals, fall prevention, equipment use, and safe mobility.
  • Document clearly and timely in PCC or similar EHR to support medical necessity, regulatory requirements, and quality care.

Training & Support: Structured onboarding, ongoing supervision by a physiatrist, and education on PDPM, rehab principles, and SNF workflow will be provided.

Preferred Skills & Experience—willing to train the right candidate

Interest in or exposure to:

  • Physical Medicine & Rehabilitation (PM&R)
  • Skilled Nursing Facilities, inpatient rehab, LTACH, or geriatric/post-acute care
  • Basics of rehabilitation health practices and functional outcome measures
  • Management of older adults/medically complex patients
  • Interpretation of common diagnostic tests and basic imaging (X-ray, CT, MRI)
  • Experience with PCC or similar electronic health record systems is a plus.
  • Strong communication and teamwork skills in a facility-based environment.
  • Working knowledge of HIPAA and clinical documentation standards.

Qualifications

  • Valid certification as a Physician Assistant (PA-C) or Nurse Practitioner (NP) from an accredited program; current state licensure required.

Pay: $100,000.00 – $130,000.00 per year
