Role OverviewWe’re seeking an experienced, adaptable, and reliable Physician Assistant for in-person home visits. This role is ideal for a provider who enjoys autonomy, meaningful patient interactions, and a flexible schedule. You’ll conduct comprehensive evaluations, follow-ups, chronic care management, and acute visits as clinically indicated.
What You’ll Do
- Perform in-person home visits across your base county
- Conduct H&Ps, follow-ups, chronic disease management, and acute care visits
- Complete documentation in the EHR within required timeframes
- Collaborate with supervising physicians and multidisciplinary teams
- Communicate promptly with the Calyx admin team
- Maintain high standards of professionalism, patient rapport, and reliability
- Participate in occasional virtual huddles or case review sessions
What You Bring
- Current Physician Assistant license in California
- Strong clinical assessment and diagnostic skills
- Experience in home health, primary care, geriatrics, internal medicine, or urgent care preferred
- Reliable transportation and willingness to drive within assigned regions
- Excellent communication and time-management skills
- Comfort with technology and EHR documentation
- Ability to work independently and maintain clinical judgment in the field
