Position Summary:This Community Rounding PA-C role focuses on delivering integrated care—combining primary medicine with advanced wound management and chronic condition support. You’ll work within a physician-led team that values patient connection and preventive care.
Responsibilities:
- Round in assigned senior living communities to provide primary and wound care
- Participate in care coordination and interdisciplinary team meetings
- Manage chronic diseases, medications, and treatment updates
- Maintain accurate, thorough clinical documentation
Qualifications:
- Licensed PA-C in Minnesota
- Experience or strong interest in geriatrics or wound care
- Reliable transportation required
