Full Job Description

Senior Health is seeking a highly skilled and compassionate Nurse Practitioner (NP), Physician Assistant (PA), or Medical Doctor (MD) to provide specialized wound care services in skilled nursing facilities (SNFs). The ideal candidate will have advanced training and certification in wound care, along with prior experience in post-acute or long-term care settings. This role involves delivering patient-centered wound management, collaborating with interdisciplinary teams, and ensuring optimal healing outcomes for residents.

Key Responsibilities

· Conduct comprehensive wound assessments, including etiology, staging, and treatment planning.

· Develop, implement, and monitor individualized wound care plans in alignment with best practices and clinical guidelines.

· Perform wound debridement, dressing changes, and other advanced wound management procedures.

· Collaborate with nursing staff, therapists, dietitians, and physicians to support holistic patient care.

· Educate facility staff on wound prevention, treatment protocols, and evidence-based practices.

· Document assessments, treatments, and progress in compliance with regulatory and facility standards.

· Participate in quality improvement initiatives related to wound management and patient outcomes.

· Provide consultation on infection control and skin integrity programs within the facility.

Qualifications

· Licensed NP, PA, or MD in good standing with the applicable state board.

· Certification in wound care (e.g., WOCN, CWCN, CWS, or equivalent) required.

· Minimum of 2 years of clinical experience, preferably in wound care, geriatrics, or skilled nursing facilities.

· Strong knowledge of wound healing physiology, advanced therapies, and evidence-based protocols.

· Excellent communication, collaboration, and clinical documentation skills.

· Ability to work independently and manage a diverse patient population across multiple facilities.

Preferred Experience

· Prior work experience in skilled nursing facilities or long-term care settings.

· Experience with pressure injury prevention and management programs.

· Familiarity with EMR systems used in post-acute care.
