Role Summary:
Position is for a licensed Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to lead our cardiology team in a transformative initiative aimed at improving outcomes for heart failure patients. - Utilize remote monitoring tools and telehealth platforms for high-risk patient surveillance.
- Educate patients and families on medication adherence, dietary guidelines, and symptom recognition.
- Coordinate referrals to ancillary services including cardiac rehab, dietary services, discharge planning, and social work
- Participate in patient education programs and workshops to promote self-care and family engagement.

- Minimum of 2–3 years of clinical experience, preferably in cardiology or critical care
- Proficiency in EMR systems and remote monitoring platforms; Epic preferred
- Strong communication, organizational, and patient engagement skills
