Family Health Matters (FHM) is a rapidly growing Federally Qualified Health Center (FQHC) committed to providing high-quality, compassionate, and inclusive care to families across Anaheim and surrounding communities. Our team provides a wide array of services including comprehensive primary care, OB-GYN, vision, behavioral health, patient navigation, and outreach/enrollment assistance. We are currently seeking a mission-driven Nurse Practitioner or Physician Assistant with a focus in OB-GYN and women’s health to join our integrated care team.
Position Overview
The NP/PA will provide comprehensive OB-GYN and women’s health services in a team-based environment. This includes prenatal care, gynecologic exams, family planning, and managing chronic and acute gynecologic issues. The ideal candidate is passionate about community health and understands the unique needs of underserved populations.
Key Responsibilities
- Deliver direct patient care in OB-GYN and women’s health, including prenatal and postpartum care, gynecologic assessments, contraception counseling, and STI management.
- Place and remove long-acting reversible contraceptives (LARC), including Nexplanon and IUDs in accordance with clinical protocols.
- Collaborate with providers, care coordinators, and support staff to provide integrated care.
- Ensure timely and accurate documentation using eClinicalWorks (eCW) electronic health record (EHR) system.
- Participate in quality improvement activities and population health strategies.
- Provide culturally competent care aligned with FQHC and PCMH (Patient-Centered Medical Home) standards.
Qualifications
- Current California licensure as a Nurse Practitioner or Physician Assistant.
- Current DEA license
- Certification in Women’s Health, Family Practice, or OB-GYN preferred.
- Bilingual in English and Spanish highly desirable.
- Minimum of 2-3 years of experience in OB-GYN or women’s health (FQHC or underserved populations experience a plus).
- BLS certification
- Familiarity with Medi-Cal and other public coverage programs.
- Strong communication, teamwork, and patient education skills.
