Full Job Description

We are seeking a Family APP to become a part of our team! You will focus on providing high quality patient care as part of a healthcare team. This Community Health Center has been a strong part of the community for multiple decades.

Your duties will include being responsible for the delivery of primary care services including preventative health and chronic disease management, as well as ongoing management of common minor and acute illnesses and emergency care.

Responsibilities:

  • Responsible for the delivery of primary medical care including assessment, treatment plan and patient instructions
  • Conduct comprehensive physical examinations and medical assessments
  • Performs all lab testing in accordance with established policies and procedures
  • Diagnose and treat acute, episodic or chronic illnesses
  • Prescribe all necessary medications
  • Develop and implement patient management policies and procedures
  • Communicate with collaborating physician or specialist regarding patient care
  • Educate patients and their families about health conditions, and treatment options.

Qualifications:

  • Current certification as an Family Nurse Practitioner or Physician Assistant
  • Ability to work independently and make sound clinical judgments
  • Proficiency in electronic medical record (EMR) systems
  • Only qualified candidates will be contacted for an interview
  • If you have less than 2 years of expereince working as a licensed NP or PA you will not qualify for this position.

Job Type: Full-time

Pay: $115,000.00 – $135,000.00 per year
