NURSE PRACTITIONER / PHYSICIAN ASSISTANT

Join UltaCare Medical, a dynamic, growing team dedicated to supporting, onboarding, and rewarding our clinicians—so they can focus on delivering exceptional care. We are currently seeking Nurse Practitioners and/or Physician Assistants to provide medical care to both short-term and long-term residents in skilled nursing and senior living facilities. In this role, you will partner with facility staff to provide proactive care for a full spectrum of chronic conditions.

RESPONSIBILITIES

· Collaborate with facility staff and off-site Medical Director to proactively manage acute and chronic conditions to minimize disease progression, lessen the risk of major acute events, alleviate side effects, and enhance quality of life

· Participate in quality improvement projects

· Independently assess urgent and routine medical needs, manage medications, and communicate care plans with patients and facility staff

· Document according to CMS and UltaCare guidelines

· Demonstrate compassion and commitment to facility staff, patients, and families

• Conduct medical evaluations, treatments, and consultations for facility residents.

QUALIFICATIONS

· Active, unrestricted license

· Demonstrated ability to independently identify patients for care, prioritize and manage scheduling, and communicate with multiple stakeholders

· Demonstrated experience providing medical care for a wide range of diagnoses and conditions

· Experience with geriatric population in post-acute settings, preferred

· Demonstrated ability to work independently

Salary: $105,000-$110,000 fulltime with uncapped monthly productivity bonus
