OverviewWe are seeking a dedicated and compassionate Primary Care Nurse Practitioner/Physician Assistant to join our healthcare team. In this role, you will provide comprehensive care to patients, focusing on preventive health measures and managing chronic conditions. You will work collaboratively with physicians and other healthcare professionals to deliver high-quality patient care in a supportive environment.

Responsibilities

• Conduct thorough patient assessments, including medical history and physical examinations.
• Develop and implement individualized care plans based on patient needs.
• Perform diagnostic tests and interpret results to guide treatment decisions.
• Administer treatments, including IV insertion and tube feeding, as necessary.
• Manage patient care in various settings, including memory care facilities and teaching hospitals.
• Participate in discharge planning to ensure smooth transitions of care for patients.
• Maintain accurate medical records and documentation for all patient interactions.
• Engage in utilization review processes to optimize patient outcomes and resource use.
• Provide education and support to patients and their families regarding health management.

Job Type: Full-time

Pay: $90,000.00 – $99,999.00 per year
