Position Overview:
We are seeking a skilled and motivated Nurse Practitioner or Physician Assistant or Medical Director with a passion for wound care to join our growing mobile team. The ideal candidate will have experience in wound care or a strong desire to specialize in the field. You will work autonomously within a collaborative environment, conducting on-site assessments and providing treatment to patients in the Virginia Beach, Norfolk, and Chesapeake areas.
Key Responsibilities:
- Provide advanced wound care evaluation, treatment, and follow-up to patients in a variety of settings.
- Perform bedside procedures including debridement, dressing changes, and wound assessments.
- Collaborate with physicians, nursing staff, and facility teams to ensure continuity of care.
- Utilize EMR systems for accurate documentation of patient encounters, treatment plans, and progress.
- Educate patients and caregivers on wound care and prevention.
- Participate in care planning and contribute to reducing rehospitalizations and complications.
Qualifications:
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Virginia.
- Wound care experience strongly preferred, but training will be provided for the right candidate.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Valid driver’s license and reliable transportation (this is a mobile role).
- Home Health Experience
