Responsibilities:

  • This is a Traditional Out-patient, Primary Care Provider opportunity. You will see all ages with heavier emphasis on the adult and geriatric population.
  • Evaluate, diagnose, treat and provide consultation to patients of all ages with acute and chronic disease including major complicated illnesses.
  • Ensures proper injury care and disease prevention, diagnosis, treatment, and recovery.
  • Prescribes and administers medications and provide patient education.
  • Requests necessary tests and follow-up visits and refers patients to specialists, as necessary.
  • Work in collaboration with a MD or DO.

Clinic Operating Hours:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Full-time, 5 days a weeks

Job Requirements

  • IL Medical license
  • Board Certified
  • FNP or PA
  • EMR Experience

Required Skills·Strong interest and passion for helping people in the community·Ability to work independently without supervision·Excellent communication skills, both written and verbal· Ability to perform debridement and other advanced wound care treatments·Willingness to learn and ability to adapt to change· Must have reliable transportation and own valid driver’s license·Must be open to performing mobile advanced wound care services

Job Types: Full-time, Permanent

Pay: $105,000.00 – $125,000.00 per year
