Who We’re Seeking:You’re a reliable, clinically confident NP or PA with a heart for geriatric care. You bring strong diagnostic skills, effective communication, and a proactive approach to managing chronic and acute conditions in skilled nursing settings. You’re also comfortable working independently, documenting efficiently, and coordinating care with facility teams.

Position Overview:This weekend position is designed for providers interested in supporting patients during transitions of care, post-hospitalization recovery, and ongoing chronic condition management. You’ll serve as an essential care partner, ensuring continuity and quality care throughout the weekend within SNF environments.

Key Responsibilities:

  • Conduct initial and follow-up assessments for residents in SNFs
  • Diagnose and manage acute conditions, exacerbations, and chronic illnesses
  • Prescribe medications and adjust treatment plans based on clinical status
  • Order and interpret labs, imaging, and diagnostic tests
  • Communicate with patients, families, and facility staff regarding care plans
  • Collaborate with attending physicians, therapists, and nursing teams
  • Support discharge planning and educate patients/families on follow-up care
  • Maintain accurate documentation in the EMR in a timely manner
  • Comply with regulatory and facility-specific protocols

Qualifications:

  • Current, unrestricted Arizona license as a Nurse Practitioner or Physician Assistant
  • Graduate of a U.S.-accredited NP or PA program (excluding Puerto Rico)
  • 1–2 years of experience in skilled nursing or post-acute care strongly preferred
  • EMR proficiency and ability to work independently
  • Excellent clinical judgment and interpersonal skills
