What We’re Looking For:
You’re a focused and adaptable Nurse Practitioner or Physician Assistant who thrives in clinically intense settings. You have strong critical thinking skills and are confident managing acutely ill or recently discharged patients. Whether you bring years of acute care experience or are a highly capable new graduate, you’re driven by results, patient safety, and clear communication.
Role Overview:
This full-time role centers on the care of high-risk patients in an outpatient setting, including individuals transitioning from hospital care, recovering from surgery, or managing multiple chronic illnesses. You’ll work closely with physicians, care coordinators, and specialty teams to provide thorough, timely, and effective clinical management.
Key Responsibilities:
- Conduct comprehensive patient assessments and post-operative evaluations
- Diagnose and manage acute medical conditions and complications
- Monitor for surgical site infections, adverse medication effects, and systemic changes
- Develop and implement evidence-based care plans
- Order and interpret diagnostic studies, including labs and imaging
- Prescribe medications within your licensure and monitor therapeutic response
- Educate patients and caregivers on treatment plans and warning signs
- Coordinate care with hospitals, surgeons, specialists, and case managers
- Maintain complete, timely, and accurate documentation in EHR
- Participate in discharge planning, follow-up scheduling, and clinical handoffs
Required Qualifications:
- Graduate of a U.S.-accredited NP or PA program (excluding Puerto Rico)
- Active Arizona license as a Nurse Practitioner or Physician Assistant (or eligibility to obtain)
- National board certification (NP or PA-C)
- Ideally 3–4 years of clinical experience in acute care, urgent care, or inpatient medicine
- Strongly motivated new grads with high-level clinical rotations are also encouraged to apply
