Job Summary:
CardioRenal Vision (CRV) is the national leader in post-acute Cardiology and Nephrology clinical care. Currently, we are looking to fill a full-time position for a qualified Nurse Practitioner or Physician Assistant to join us in the Tacoma area. As a clinician specialist with CRV, you will manage both the complex acute skilled patients who recently admitted to the SNF, and the long-term care patients who we take a more preventative approach with. Our goal is to treat in-place at the SNF if possible to reduce unnecessary readmission back to acute.
You will work closely with a supervising cardiologist and nephrologist. We also have a training and mentorship program for newly hired providers to ensure you have adequate clinical support. Requirements:
Experience in the Skilled Nursing setting as a nurse or APC is helpful. One year of cardiology or nephrology experience preferred.
Primary Responsibilities:
- Communicate and collaborate with facility leadership including the Director of Nursing and Administrator.
- Communicate and collaborate with the primary care team at the facility including the Medical Director and NP/PA.
- Be open to providing education and training to the facility’s nursing team.
- Achieve encounter goals on a monthly basis.
- Complete all documentation in a timely manner.

Job Type: Full-time
Pay: $135,000.00 – $210,000.00 per year
