Full Job Description
- Practice with a multi-disciplinary team of health care providers that includes, Physicians, APC’s, RN Case Managers, Social Workers, and clinic staff.
- Our model is designed to provide a comprehensive, coordinated model of care to a predominantly Medicare-aged population.
- Primarily manage care in the patient’s home; however, care may require an office or other facility setting depending on the needs of the patient.
- Diagnose and treat both acute and chronic conditions such as diabetes, hypertension, kidney disease and other chronic medical conditions.
- Treat acute conditions such as minor injuries and other acute medical conditions such as infections or exacerbations of chronic diseases.
- Facilitate patient and family decision making regarding treatment decisions and end of life care and serious illness discussions.
- Prescribe pharmacological and non-pharmacological interventions.

Required Qualifications:
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
- Current Licensure through the Texas Board of Nurse Examiners OR Licensed Physician Assistant by the Texas State Medical Board
- Valid DEA license or the ability to obtain one prior to employment
- Comfortable managing a primarily Medicare-aged patient population
- 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant
- Reliable transportation and a valid driver’s license
Preferred Qualifications:
- Home health experience
- Bilingual (English/Spanish) fluency
- A working understanding and support of a managed care environment
The salary range for this role is $110,000 to $166,500 per year based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
