POSITION SUMMARY: The APP is a board-certified Family Nurse Practitioner or Physician Assistant with combined specialty certification in women’s health. The APP conducts comprehensive health assessments, demonstrates a high level of autonomy and expert skill in the diagnosis of complex health responses of individuals and families. Additionally, as a women’s health care provider, the APP will treat the health care needs of women throughout their lifespan, including prenatal and postnatal care working closely with the OB/GYN specialist in obstetrics and gynecology patient care. The APP will work directly under the supervision of a physician, share a collaborative agreement and practice independently.
ESSENTIAL DUTIES:
1. Develops, monitors and alters a comprehensive health care treatment program for individuals and families through
a. Performing assessment/data collection in an on-going patient-centered, systematic manner, focusing on physiological, psychosocial, cultural, spiritual and cognitive status, patient/family education, family involvement, and patient advocacy
b. Discriminating between normal and abnormal findings in planning treatment interventions.
c. Discussing significant abnormal findings with other health care professionals.
d. Synthesizing complex clinical data in order to formulate, implement and evaluate a treatment plan for individuals.
e. Instructing patients in the acquisition of information related to health and wellness.
f. Initiating and implementing appropriate consultations and referrals
2. Practices under the standard procedure established for the APP role consistent with Company policy and in accordance with all legal requirements.
a. Conducts age-appropriate assessments of patients, including history taking and physical examination consistent with department protocol within the OB/GYN and post-partum patient population.
b. Performs or assists with Colposcopies, IUDs and ultrasounds
c. Provides back-up provider coverage when OB/GYN physician is out of clinic (vacation, sick, etc.).
d. Maintains and expands current knowledge base regarding advanced clinical practice in the area of OB/GYN.
3. Consults with supervising physician and models interdisciplinary collaboration with other health care professionals to ensure continuity of care and optimal patient outcomes.
EDUCATION & QUALIFICATION REQUIREMENTS:
· Master of Science Degree in Nursing from an accredited school of nursing
· Minimum 1-3 years’ experience in ambulatory care.
· Experience with serving the medical needs of minority populations.
· Strong community health/public health orientation.
· Experience working with interdisciplinary teams.
Certifications/Licensures:
· Valid Indiana ANP/CSR license.
· Valid Indiana RN license, or valid Indiana PA license.
· Current national certification as an Advanced Practice Nurse, or current national certification as a Physician Assistant-Certified or PA-C.
· Certification in maternal care (prenatal, intrapartum and postpartum care).
· Current BLS certification.
· Valid, or ability to obtain, DEA certification.
