Position: Nurse Practitioner / Physician Assistant
Location: Middleton, WI 53562
Duration: ASAP – 05/30/25 (Possible extension or conversion to Perm.)
Pay: $80 – 85/hr
Work Schedule:

Monday & Tuesday: 6:30 AM – 4:30 PM

Wednesday & Thursday: 11 AM – 7 PM

Friday: 8 AM – 12 PM
Overview:

The Health center supports a Patient-Centered Medical Home (PCMH) model
-
Integrated services include primary care, behavioral health, physical therapy, lab services, and provider dispensing
-
Utilizes EPIC EMR for patient documentation
-
Primary/Occupational Care employee clinic
Essential Requirements:
-
3+ years of Family practice experience is preferred.

EPIC EMR experience is a PLUS
-
Must have an active NP License in Wisconsin
-
Must have an active BLS or willing to obtain post-offer
-
DEA License preferred
-
Must have a clean background
