Job OverviewWe are seeking a dedicated and skilled Primary Care Provider (Nurse Practitioner or Physician Assistant) to join our healthcare team. In this role, you will provide comprehensive care to primarily Cantonese-speaking patients with a focus on geriatrics and elderly, focusing on chronic disease management, preventive health, and proactive, risk-based service. You will work collaboratively with the senior physician partner at the practice, who is also a native Cantonese speaker with decades of primary care expertise. You will start with a full geriatric patient panel, and together with your physician partner you will ensure the highest quality of patient care in a clinic setting.
Responsibilities
- Conduct thorough patient assessments, including physical examinations and health histories.
- Develop and implement individualized treatment plans based on patient needs.
- Provide acute care for patients with a variety of health concerns, including but not limited to ophthalmology and gastroenterology issues.
- Educate patients and their families about health management, disease prevention, and wellness strategies.
- Monitor patient progress and adjust treatment plans as necessary.
- Maintain accurate and up-to-date patient records using electronic health record systems.
- Ensure compliance with infection control protocols and best practices in clinical settings.
- Collaborate with multidisciplinary teams to enhance patient outcomes.
- Provide care for infants and pediatric patients, addressing their unique healthcare needs.
Qualifications
- Nurse Practitioner or Physician Assistant credentials and the corresponding state licensure in good standing in California
- Strong knowledge of anatomy and physiology relevant to primary care practice.
- Experience in primary care in a hands-on clinical setting is preferred; familiarity with value-based care environments is a plus.
- Proficient in using electronic health record systems.
- Fluency in Cantonese Chinese is required; Mandarin Chinese optional.
- Excellent communication skills, both verbal and written, with the ability to educate patients effectively.
- Strong organizational skills and attention to detail in managing patient care documentation.
- Ability to work collaboratively within a team-oriented environment while also demonstrating independence when necessary.
- Previous experience or training in pediatrics is highly desirable.
