Full Job Description
Patient Care:
- Conduct thorough patient assessments, including history-taking, physical exams, and diagnostic evaluations for adults.
- Diagnose and treat acute and chronic medical conditions, including but not limited to diabetes, hypertension, COPD, and cardiovascular disease.
- Order, interpret, and follow up on diagnostic tests, lab results, imaging, and other diagnostic procedures.
- Provide education to patients and families regarding treatment plans, medication management, and lifestyle modifications.
- Prescribe medications, therapies, and other treatments as needed, in accordance with the scope of practice and applicable state regulations.
Preventative Care & Wellness:
- Perform annual wellness visits, physical exams, and preventive screenings in accordance with evidence-based guidelines.
- Develop and implement individualized care plans for patients with chronic diseases to improve long-term health outcomes.
- Promote and educate patients about healthy lifestyle choices and disease prevention strategies.
- Provide immunizations, routine screenings (e.g., cancer screenings), and counseling on preventive healthcare topics.
Collaborative Care:
- Collaborate with physicians, nurses, medical assistants, and other healthcare team members to ensure continuity of care and optimal patient outcomes.
- Participate in case discussions, patient management meetings, and other interdisciplinary team activities.
- Consult with specialists and refer patients as needed for specialized care.
Documentation & Compliance:
- Accurately document patient assessments, plans, progress notes, and procedures in the electronic medical record (EMR).
- Ensure compliance with all relevant state and federal healthcare regulations, including HIPAA and safety standards.
- Maintain up-to-date knowledge of best practices, clinical guidelines, and emerging healthcare trends.
Patient Support & Communication:
- Establish and maintain positive, professional relationships with patients to ensure high levels of patient satisfaction.
- Effectively communicate with patients regarding their health concerns, treatment options, and care instructions.
- Provide follow-up care and check-ins as necessary to ensure patient progress and health goals are being met.
Job Type: Full-time
Pay: $120,000.00 – $130,000.00 per year
