Mid-Level Primary Care Provider (Nurse Practitioner / Physician Assistant)

Patient Care:

Conduct thorough patient assessments, including history-taking, physical exams, and diagnostic evaluations for adults.
Diagnose and treat acute and chronic medical conditions, including but not limited to diabetes, hypertension, COPD, and cardiovascular disease.
Order, interpret, and follow up on diagnostic tests, lab results, imaging, and other diagnostic procedures.
Provide education to patients and families regarding treatment plans, medication management, and lifestyle modifications.
Prescribe medications, therapies, and other treatments as needed, in accordance with the scope of practice and applicable state regulations.

Preventative Care & Wellness:

Perform annual wellness visits, physical exams, and preventive screenings in accordance with evidence-based guidelines.
Develop and implement individualized care plans for patients with chronic diseases to improve long-term health outcomes.
Promote and educate patients about healthy lifestyle choices and disease prevention strategies.
Provide immunizations, routine screenings (e.g., cancer screenings), and counseling on preventive healthcare topics.

Collaborative Care:

Collaborate with physicians, nurses, medical assistants, and other healthcare team members to ensure continuity of care and optimal patient outcomes.
Participate in case discussions, patient management meetings, and other interdisciplinary team activities.
Consult with specialists and refer patients as needed for specialized care.

Documentation & Compliance:

Accurately document patient assessments, plans, progress notes, and procedures in the electronic medical record (EMR).
Ensure compliance with all relevant state and federal healthcare regulations, including HIPAA and safety standards.
Maintain up-to-date knowledge of best practices, clinical guidelines, and emerging healthcare trends.

Patient Support & Communication:

Establish and maintain positive, professional relationships with patients to ensure high levels of patient satisfaction.
Effectively communicate with patients regarding their health concerns, treatment options, and care instructions.
Provide follow-up care and check-ins as necessary to ensure patient progress and health goals are being met.

Job Type: Full-time

Pay: $120,000.00 – $130,000.00 per year
