About the Job
At our rapidly growing practice, Cardiovascular Specialists of New England, we are seeking a highly motivated Nurse Practitioner or Physician Assistant to join our team! You will be responsible for the assessment and treatment of cardiology patients in the inpatient and outpatient settings. You will collaborate with the cardiologists in delivering cardiology treatment plans, patient education and care coordination. You will report directly to the supervising physician.
What you’ll do:
- Perform Cardiology inpatient and outpatient consultations and follow-ups
- Provide peri-procedural patient management for Interventional cardiology, peripheral, electrophysiology and structural procedures
- Responsible for the assessment and treatment of cardiology patients, including development and execution of individualized care plans
- Educate patients and their families on disease prevention, treatment options and self-care. Provide and promote effective and compassionate care for patients and their families
- Document patient history and physicals findings, assessment and plan promptly in the electronic medical record patient chart.
- Determine and complete orders including diagnostic testing, labs and medications within the EMR in accordance with institutional policy and procedures
- Assess pre and post procedure patient status, discuss procedural findings, answer appropriate questions and provide educational and emotional support to patients and family members. Prescribe medications and arrange follow up care
- Communicate with referring physicians and collaborate with other healthcare professionals to coordinate patient care and ensure optimal outcomes
- Participate in quality improvement initiatives to enhance patient care delivery
- Promptly submit billing information
What you’ll need:
- Graduate of an accredited Nurse Practitioner program or Physician Assistant program
- Three years or more as a Nurse Practitioner or Physician Assistant preferred (cardiology experience strongly preferred)
- Licensed by the State of NH as an APRN or PA
- ACLS (Advanced Cardiovascular Life Support) and BLS certification required
Work Location:
You will work out of the Londonderry, New Hampshire office.
