Baystate Health, western Massachusetts’ premier healthcare provider and home to the University of Massachusetts Chan Medical School – Baystate, is seeking a Physician Assistant or Nurse Practitioner to join High Street Community Health Center.
Position Highlights:
- Join a well-established and dedicated team of physicians and advanced practitioners providing outpatient innovative and compassionate, comprehensive, and community-based care to the inner-city, predominantly Hispanic/Latino community. NACQ patient centered medical home.
- Care for an adult patient population with complex needs and health disparities in an academic setting.
- Opportunity to work closely with residents from the Baystate’s Internal Medicine Residency Program and Advanced Practice Students from local programs.
- Multi-disciplinary team with on-site support from: certified nurse-midwives, behavioral health services, opioid addiction treatment program, HIV medical care and support services, laboratory services, pharmacy, and social services.
