POSITION HIGHLIGHTS:
- Join our inpatient cardiac surgery team that provides specialized care for patients on the Cardiac Surgery Step-Down floor at Baystate Medical Center in Springfield MA. Our Heart & Vascular Service Line includes heart failure physicians, vascular surgeons, cardiologists, cardiac surgeons, electrophysiologists, anesthesiologists, radiologists, and advanced practitioners across 4 hospitals and multiple community sites.
- This position involves daily rounding, patient evaluation, care coordination, and discharge planning in collaboration with our multidisciplinary team. Facilitate smooth transitions of care between the ICU, step-down and discharge settings.
- Ensure the delivery of high-quality, evidence-based care to optimize patient outcomes and prevent hospital readmissions. Serve as the primary liaison between the care team and patient/families, providing education.
- Engage in quality improvement initiatives to enhance outcomes and optimize workflow processes.
- This position is full time, Monday-Friday day shift.
- Qualifications: Physician Assistant or Acute Care Nurse Practitioner with experience in cardiac surgery, cardiology, or critical care.
