Full Job Description

Cornerstone Health Community (CHC) is a unique primary care and functional medicine practice that promotes a strong patient and healthcare team partnership. Located near the Denver Tech Center in Centennial, CHC offers leading edge functional medical solutions in a Family Medicine private practice setting.

CHC is growing! We are currently seeking a full time Family Medicine Physician Assistant to join our provider team. The PA-C will team with our physician and APPs in serving our patient’s primary healthcare needs.

Scheduled hours are Tuesday – Friday, 7:30am – 5:30pm

In addition to a rotating call schedule, the Physician Assistant will teach group health visits via telehealth twice a month.

Providing exceptional healthcare begins with the health and well-being of the healthcare team. We are closed on Saturday, Sunday and Monday to provide our team consecutive days to rest, enjoy CO living and worship, if they choose, with family and friends.

Job Responsibilities

  • Practices medicine in collaboration with a physician in accordance with accepted practice standards
  • Obtains pertinent medical history, performs an appropriate physical examination, and reviews relevant laboratory results or studies to assess the patient’s medical, physical, and psychological status
  • Formulates an accurate and appropriate diagnosis based on elicited data, and includes differential diagnoses as appropriate
  • Develops and implements treatment plans that are aligned with the practice including orders for medication and/or other treatment modalities, the need for additional testing, referral to other specialists, and follow-up provisions
  • Performs procedures within the limits of their expertise and in accordance with the State Regulations and clinic practices
  • Educates and counsels patients and their families regarding their treatment plan, including disease prevention and health promotion
  • Documents all examination data, diagnostic impressions, appropriate codes, and other patient care activities in the medical record, including the name(s) of any consulted physicians or other providers
  • Performs other clinical/administrative duties as directed
  • Ensures that the patient is the primary focus of their actions; develops and sustains productive and compassionate patient relationships
  • Positively influences practice culture by modeling traits that promote respect for people, embrace continuous improvement, and a team approach to exceptional patient care.
  • Brings fresh ideas, unique strengths, creative problem solving to the team.

Job Requirements

  • Two + years experience as a PA in a Family Medicine setting, will consider Internal Medicine experience, may consider new graduates
  • Current unrestricted licensure as a Physician Assistant in the State of Colorado.
  • Current and continued certification by the National Commission for the Certification of Physician Assistants (NCCPA)
  • Graduation from a program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA)
  • National Provider Identification (NPI) required at time of hire
  • Active DEA Registration Certification by start date.
  • Excellent verbal and written communication and interpersonal skills

Pay: $100,000.00 – $135,000.00 per year
