OverviewWe are seeking a highly motivated and enthusiastic advanced care practitioner, either a Nurse Practitioner or Physician Assistant to join our well established Internal Medicine Practice in the East Bay, serving a diverse patient population across a board age range from 18 to over 100 years of age. We have two experienced internists who will provide a collaborative and collegial atmosphere. The ideal candidate will possess a strong clinical background and demonstrate a commitment to providing high-quality patient care. This role involves working collaboratively with physicians and other healthcare professionals to assess, diagnose, and treat patients across various settings, including telehealth and home care.
While prior experience with EPIC is not required, it is highly beneficial. Our practice offers flexible scheduling options, ranging from 3.5 days to full time, combined telemedicine and in person visits.

A valid California Medical license is required with a projected start date of mid to late March 2025
Responsibilities
- Conduct comprehensive patient assessments, including physical examinations and health histories.
- Develop and implement individualized care plans based on patient needs.
- Administer injections and perform procedures.
- Provide family planning services and education to patients.
- Triage patient concerns effectively to determine the urgency of care required.
- Utilize telehealth platforms to deliver remote patient consultations and follow-ups.
- Monitor patient progress and adjust treatment plans as necessary.
- Educate patients on health maintenance, disease prevention, and treatment options, including botulinum toxin treatment when applicable.
- Maintain accurate medical records in compliance with standards of practice.
Experience
- Valid Nurse Practitioner license in the state of practice.
- Previous experience is preferred.
- Proficiency in patient assessment techniques and familiarity with administering injections is essential.
- Experience with triage processes and managing diverse patient populations is highly desirable.
