First Choice Health Centers is seeking a Nurse Practitioner or Physician Assistant experience for a practice opportunity in Manchester, CT. This position is accountable for independently performing a full range of tasks in delivery of direct patient care in an outpatient setting.

- The salary range for this position is $110,000 – 145,000. Pay is based on several factors including but not limited to years of related work experience, certifications, etc.

Experience and Training:
- Registered Nurse, and completion of an accredited Nurse Practitioner program or a Graduate of an accredited Physician Assistant program.
- A current license to practice as a NP or PA in the state of Connecticut.
- Appropriate Board Certification
- At least one year of experience working in a primary care setting
- A commitment to serving diverse and vulnerable populations with compassion and respect.
- Excellent communication, interpersonal, and organizational skills.
- Connecticut Controlled Substance license and DEA license
- BLS Certification through the American Heart Association or American Red Cross.
Standard Job Duties:
- Evaluates, assesses patient illness/injuries.
- Performs physical examinations, and special examinations.
- Takes medical histories.
- Develops plans of care.
- Provides routine and illness care.
- Orders medications; orders diagnostic tests; and evaluates results.
- Assumes responsibility for patient follow-up, coordination, and referrals to other health care providers as necessary.
- Maintains patient medical records.
- Provides patient counseling/education.
- Participates in medical student and health professional teaching.
- Performs related duties as required.
