Full Job Description

First Choice Health Centers is seeking a Nurse Practitioner or Physician Assistant experience for a practice opportunity in Manchester, CT. This position is accountable for independently performing a full range of tasks in delivery of direct patient care in an outpatient setting.

  • The salary range for this position is $110,000 – 145,000. Pay is based on several factors including but not limited to years of related work experience, certifications, etc.

Experience and Training:

  • Registered Nurse, and completion of an accredited Nurse Practitioner program or a Graduate of an accredited Physician Assistant program.
  • A current license to practice as a NP or PA in the state of Connecticut.
  • Appropriate Board Certification
  • At least one year of experience working in a primary care setting
  • A commitment to serving diverse and vulnerable populations with compassion and respect.
  • Excellent communication, interpersonal, and organizational skills.
  • Connecticut Controlled Substance license and DEA license
  • BLS Certification through the American Heart Association or American Red Cross.

Standard Job Duties:

  • Evaluates, assesses patient illness/injuries.
  • Performs physical examinations, and special examinations.
  • Takes medical histories.
  • Develops plans of care.
  • Provides routine and illness care.
  • Orders medications; orders diagnostic tests; and evaluates results.
  • Assumes responsibility for patient follow-up, coordination, and referrals to other health care providers as necessary.
  • Maintains patient medical records.
  • Provides patient counseling/education.
  • Participates in medical student and health professional teaching.
  • Performs related duties as required.
